Community Services: Targeted Case Management

Mahaska County Case Management (MCCM) provides support and advocacy to people with Intellectual Disabilities, Brain Injury, and Developmental Disabilities with a focus on individual strengths and desires.

MCCM is a state accredited Targeted Case Management provider for persons with Intellectual Disabilities, Brain Injuries, and other Developmental Disabilities. TCM is an individualized service to assist persons in planning and attaining goals to live more productive lives. The Case Manager will help an individual with the following:

Identifying needs and desires

Referring to appropriate services

Linking to community resources

Accessing and securing funding

Monitoring and coordinating services

Enhancing natural supports

Completing required paperwork in order to obtain and maintain services

Once you have a case manager, you will be the leader of a team who will discuss what services and supports you will need to reach your goals.


To be eligible to Targeted Case Management services, an individual must:

Be enrolled in Medicaid (TXIX)

Have a documented diagnosis (Intellectual Disability, Brain Injury, or Developmental Disability)

Have a need for coordination of services and supports

Be willing to be involved in planning and attaining goals identified plan developed by the individual and their team
